A few helpful hints to make the referral process easy.
Thank you for choosing Sleep Solvers. Our staff is dedicated to providing high quality care and exceptional customer service to improve the health of your patients. We are also committed to making sure that the referral process is easy and convenient for you and your staff. Here are some helpful hints to consider:

•• The easiest way to make a referral is by calling us directly at 561-955-0028. Make sure to have all of your patient’s information ready to give over the telephone including, full name, address, relevant telephone numbers, date of birth, insurance information, etc. Please also be ready to give applicable information about the referring physician.

•• After we receive your patient’s information, we will call your patient directly to schedule an appointment for the Sleep Study. Once the appointment has been set, Sleep Solvers will send a New Patient Welcome Kit to your patient. This kit includes important information, preparation details and applicable forms for your patient to fill out prior to their appointment.

•• Once the Sleep Study has been conducted, Sleep Solvers will send our analysis and findings to the referring physician within 72 business hours. Once you receive the results, the referring physician or clinician should schedule a follow-up appointment to review the results of the Sleep Study with the patient.

•• If further testing or treatment is required, at the request of the referring physician Sleep Solvers is happy to provide the appropriate follow-up and treatment options.

•• We have provided you with copies of a Sleep Study Referral and Order Form that you may fax to 561-955-9141 rather than telephone in your referral. The form includes all of the necessary information that we would need to schedule your patient’s sleep study.

•• If faxing in a Sleep Study Referral and Order From, once Sleep Solvers receives your form, we will call your office to confirm receipt and then schedule an appointment for a Sleep Study with your patient directly. From there our process follows the same steps as detailed when calling in a referral.

If you are missing forms or information, please contact our office at 561-955-0028 and we will fax you the appropriate forms. We are also happy to provide brochures, appointment cards or other materials from Sleep Solvers to make the referral process easier for you.
